Instructions: Please list your nominees in order beginning with your highest-rated nominee. You must have judged a minimum of five shows at a minimum of three different theatre companies or schools in order to make nominations. The maximum number of nominations you may make is three times the number of shows you judged (if you judged 5 shows, you can make up to 15 nominations). The minimum number of nominations you must make is twice the number of shows you judged (if you judged 5 shows, you must make at least 10 nominations).
For each nomination, check the appropriate category below the name of the Show or the appropriate categories for the individual Actor Awards. For example, if it was an award for Supporting Actress in a Comedy or Drama who was 15 years old, you would check “Supporting,” “Actress,” “Play,” and NOT check “13 or Under.” If it is an “Ensemble” award, only check the “13/under” box to the right of the Ensemble box if the majority of the cast and the majority of principal players were age 13 or under. If the award is for Outstanding Production or Ensemble, you do not need to fill in the “Name” field. Be careful to spell the name correctly as printed in the program.
